Learning Objectives:
1. Describe the approaches to opioid dosing for both opioid-naïve and opioid-tolerant patients
2. Recognize when it is necessary to consider opioid titration and how to appropriately titrate the opioid
3. Discuss conversion between different opioid analgesics, dosage formulations and routes of administration, given a pain management case scenario.
Pharmacists are often the most utilized resource when it comes to making recommendations with regards to opioid rotation and calculating conversions; however, it is important that other members of the healthcare team be aware of the considerations and practical approaches when switching and converting to another opioid regimen.  This session aims to empower non-pharmacy team members when converting opioids and promote effective collaboration with pharmacy staff. Specifically, we will explore various clinical scenarios in which to consider opioid rotation, appropriate dosing for both opioid-naïve and opioid tolerant patients and proper opioid titration, while polishing up one's mathematical conversion skills!
