1. Describe the specific issues and needs associated with the start-up of a diagnosis-specific hospice program and how to best overcome obstacles
2. Detail the piloting and growth stages of a diagnosis-specific program’s development and how to maximize partnerships and opportunities while addressing setbacks
3. Distinguish the characteristics and benefits of a well established diagnosis-specific program and recognize all the benefits this can bring to the hospice, providers and patients and families.
Diagnosis-specific hospice programming – specialized hospice care tailored to the unique needs of distinct diagnostic groups – is a tremendously effective way to increase access for patients, improve care, build partnerships with fellow providers and create growth for hospices both large and small.  This presentation will explore three such programs: cardiac focused LifeBeat, dementia and debility focused Living with Dementia and a respiratory-focused Breath. Each of these programs is in a different stage of development, ranging from well-established and tested to the early formative steps. By examining the nuts and bolts of these programs, participants will be able to adapt these concepts to their own hospices, learning the development needs over time.
