Walking the Pediatric Care Continuum: A New model for Pediatric Inpatient Palliative and Hospice Services

Bereaved families have identified a consistent healthcare provider’s presence throughout their inpatient care as an indicator of receiving ‘good care.’ However, the majority of current palliative and hospice models often require transition among settings and teams. Launched in 2012, Hospice of the Bluegrass and Kentucky Children’s Hospital partnered to create a model to, in essence, walk with these patients and families throughout their illness trajectory. A single, interdisciplinary team was introduced to provide this dynamic, continuum of care service model: palliative consultation AND general-inpatient hospice level of care. This session will highlight strategies surrounding the model’s formation; its fiscal, clinical and quality benefits to both organizations; as well as staff training mechanisms.
