Fostering Exceptional Team Performance through Best Practice Care Steps

3. Describe the positive practice results from anchoring a system that supports best practice care steps, interdisciplinary communication, regulatory requirements, outcomes benchmarking and clinician ease of use
In late 2011, Suncoast Hospice launched new initiatives intended to anchor consistent best practice care steps with patients and families.  Exceptional patient and family care was the end essential outcome of this project therefore patient and family goals, wishes and needs drove the process redesign.  Through the initial planning and implementation phases, best practice care steps were redefined and clarified, electronic medical record forms were redesigned to prompt clinical best practices and collaborative team communication, quality benchmarking outcomes were defined, regulatory requirements embedded and all documentation streamlined for ease of use by hospice clinicians.  During this session, faculty will review the care steps, the implementation with the interdisciplinary teams and share the positive practice results.
