This workshop will focus on how a large for profit organization and a Midwest University College of nursing partnered to develop evidence-based practice outcomes.  Expert nursing leadership and nurse scientists developed a community-based transitional care model.  This model includes a GNP led interdisciplinary approach to palliative care across the patient’s continuum of care needs and care settings.
