Continuum of Care is when community providers work together to provide seamless coordinated care to address the comprehensive needs of patients/families.  In some communities one or two agencies offer a range of services along the continuum, while in others, multiple organizations collaborate.  In either model, how patients are transitioned to palliative care is explored throughout the disease trajectory. Since hospices are the experts in serious and life-limiting illness in their communities, it is imperative for providers to integrate the essential components of hospice and palliative care into all care settings and delivery systems.  This session will help you identify innovative approaches that expand hospice and palliative care services.  NHPCO’s Continuum of Care Assessment Tool and Guide will be highlighted plus specific strategies and resources to help meet the unique needs of patients and families in your community.
