Providing pediatric care in a hospice program necessitates a shift towards collaborative practice, which is challenging for clinicians accustomed to working independently. This is compounded in a setting where patients and clinicians from multiple cultural, ethnic and religious backgrounds are represented. The Family-Centered Care model has become imperative with an emphasis on interdisciplinary collaboration to facilitate the critical support families require. While the team focus remains on the family unit, the experience of loss is not unique to patient and family alone. This session will: utilize case presentations to illustrate clinical encounters; examine effective interventions, strategies and support; address the obstacles in providing care; and discuss approaches to team-care.

1. Describe ways that ambiguous loss affects providers of the pediatric IDT
2. Identify and anticipate markers prior to disruption of team dynamic
3. Discuss approaches to facilitate team-care (as opposed to self-care) to decrease compassion fatigue and capitalize upon one another’s strengths
