Become immersed in a heart-warming story of a ventilator dependent infant with complex congenital heart disease whose family never gave up on their wish to bring him home. Learn how an inpatient palliative care team worked closely with a pediatric hospice to orchestrate a last minute, unconventional discharge plan so the baby could go home to spend what time he had left with his parents and siblings. Expect the unexpected in this story of the love of a family and what no one thought was possible.

Learning Objectives:
1. Identify non-traditional patients with life-limiting conditions who are appropriate for pediatric home based hospice care
2. Assist families in identifying goals of care
3. Identify the steps necessary to successfully discharge a complex pediatric patient to home based hospice care using a multidisciplinary approach
