3. Identify validated tools for measuring outcomes in social work at the patient level
Outcome evaluation and evidenced-based practice are now a significant focus for hospices. An outcome-driven model of social work would ask how meaningful contributions are defined and measured, what evidence demonstrates the value of these contributions and what systems are needed to ensure favorable outcomes across palliative care teams and organizations. This presentation will explore what to measure, how to measure it and why it is imperative to do so.
