2. Discuss outcomes using the LEAN method in collaboration with a home health agency to improve FEHC scores related to HME
3. Describe opportunities for collaboration between a hospice organization and a health system to provide inpatient palliative care consultation
4. Identify positive outcomes from clinical collaboration between a hospice inpatient facility and the NC Eye Bank
There are many opportunities to collaborate outside of your organization and work together to achieve positive outcomes in quality, service and/or cost. This session will identify three successful collaborations: working with a home health agency to improve Family Evaluation of Hospice Care (FEHC) scores involving Home Medical Equipment using the LEAN Continuous Improvement process; working with a consultant to evaluate and improve a palliative care partnership between a hospice and a large health system; and collaborating with the state eye bank to educate hospice staff about the importance and value of eye donation in an inpatient hospice facility.
