1. Describe the financial challenges of a comprehensive community based palliative care based on the NCP/NQF quality program guidelines
2. Discuss practical solutions to challenging problems faced in palliative care
3. Share organizational initiatives that lead to improved care, reduced cost and increased revenue for palliative care services
Community based palliative care programs struggle to achieve financial sustainability.   In this session, faculty will share lessons learned in developing the financial model for a Circle of Life Award winning comprehensive palliative care program.  Participants will learn how leadership structure, administrative support, going “virtual”, billing and coding education, a mentoring program, PQRI, and a “special visit design” helped to improve services.  In addition, faculty will describe tools to calculate the proper support for palliative care from collaborative hospitals and hospices and share innovative strategies for raising funds.
