The Centers for Medicare and Medicaid Services have become increasingly concerned about hospital readmissions, and hospitals are responding with efforts to reduce 30-day readmissions. Although these efforts have involved a wide range of interventions and models for care, hospices have not been well-represented.  This presentation will describe the successful efforts of one hospice to partner with a major academic medical center to reduce 30-day readmission by ensuring that patients receive care that is consistent with their goals.
