Learning Objectives:
1. Create a system of psychosocial documentation that adds at least one observation of decline related to the hospice diagnosis
2. Describe at least three symptoms all psychosocial staff can assess in documenting patient decline
3. Audit at least one clinical outcome in psychosocial documentation related to patient decline
This presentation will assist managers in teaching and holding psychosocial staff accountable to documenting professional clinical outcomes while simultaneously painting the picture of the declining hospice patient. Due to increased audits, all staff must document the hospice eligibility each visit. This session will help managers assist psychosocial staff in improving their ability to document within their scope of practice and patient appropriateness for hospice care.
