3. Clarify the opportunities for collaboration between hospices and PACE programs
This session describes opportunities presented by healthcare reform to expand hospice and palliative care providers' role in caring for patients while minimizing hospital re-admissions and emergency care. Specific strategies to be implemented by providers that support the accountable care structure of the hospitals, primary care physicians and long term care facilities with which they work will be presented.
