This session will focus on the development, implement and evaluation of transitions in care program. Presenters will describe the process of applying for and receiving the CMS Community Care Transition Project Demonstration. The organizational, operational and finance issues in implementing a transition program. and how KATS collaborates and contracts with other funding sources will be discussed.
