1. Identify barriers and collaboration issues that arise between hospices and LTC facilities
2. Examine the requirements and differences between the Hospice and LTC conditions of participation when hospice services are provided to residents of LTC facilities
3. Discuss the fourteen items that need to be updated as part of every Hospice/SNF/NF contract
This presentation will provide an in-depth examination of the barriers and collaboration issues that impact the provision of hospice and palliative care to nursing home residents from a regulatory, compliance, and quality of care perspective.  Discussion will include a comparison of requirements under the conditions of participation for hospices and the new LTC regulations governing the hospice SNF/NF relationship, contracting requirements, reporting of abuse, neglect and mistreatment, reimbursement pitfalls, continuous care issues, competing roles of the medical directors, and differences in patient goals, admissions and election criteria.
