Community-Based Palliative Care (CBPC) is a high-quality, cost-effective system of care delivery that meets the Institute for Healthcare Improvement's (IHI's) Triple Aim goals of improving the patient experience of care, improving the health of the population and decreasing healthcare costs. Despite this, the use of CBPC is not yet widespread in the American healthcare continuum. This presentation makes the case for the value of CBPC and presents models for its incorporation into the American healthcare establishment.
