Establishing a Bridge Program between Hospice and Home Health

This session describes a bridge program between home health and hospice using a patient-centered approach to determine the patient's goals of care so that the right care is provided, in the right setting and at the right time. Through the use of a case study, faculty will provide participants will insights into how to establish a successful bridge program.

1. Describe the components of a bridge program
2. Identify patient characteristics that will indicate a different model of care is indicated
3. Describe necessary processes to facilitate a smooth patient transition
