Learn how to create a successful shared savings plan that bridges hospice and palliative care organizations with Accountable Care Organizations (ACOs). Discover the many partnership benefits, including new revenue streams, reduced patient care costs, increased market growth, patient access and improved quality of care. In this session, you will be able to ascertain facts on how Gulfside Hospice and Pasco Palliative Care applied the Lean Six Sigma model to define, measure, analyze, improve and control favorable outcomes for success.
