Not long ago palliative care was available only through the Medicare hospice benefit for the terminally ill; now many hospices and other palliative care providers are rising to the challenge of building community-based services for patients not yet eligible for hospice, including home-based palliative care services. Several home-based palliative care programs across the country are delivering this service to their patients. This session will highlight four programs that serve as examples for others who want to implement and sustain this critical service in their own organization.
