A major challenge facing today's hospice operations is how to serve patient or family's needs efficiently and effectively after business hours and on the weekends. This issue becomes further compounded by the challenges of finding excellent hospice nurses to serve a steadily growing patient census. This session will present an innovative solution to ensure 7-day-a week dedicated RN support, address common patient care problems, and provide quality and financial metrics to validate the models success.
Learning Objectives:
1. Discuss and relate the events that led to a shift in the Case Manager staffing model at Cornerstone Hospice
2. Explain how an organization's current RNs can be used to staff an alternative case manager model
3. Describe the benefits of providing 7-day case management combined with data analytics
