When we provide medications for a patient with a serious illness, we are entering into an implied contract to optimize outcomes, while minimizing harm. Benefits and harm may be clinical, economic or humanistic. This session will provide concrete strategies that can be implemented immediately in hospice and palliative care.

Learning Objectives:
1. Characterize the “window of opportunity” to maximize clinical benefit and value for a medication
2. Identify “high risk” medications and risk mitigation strategies designed to minimize undesirable outcomes in hospice and palliative care
3. Describe medication management practices designed to optimize patient, family and caregiver confidence and competence, which aim to also enhance CAHPS survey results
