Time, energy and mental focus are often limited for hospice patients who wish to leave a memorable legacy with family members. Latest scientific and biological research in narrative medicine has identified a powerful format that can be mastered easily and quickly for creating a meaningful life record in story. This simple formula for storytelling can be adapted to various levels of cognitive ability and easily preserved for generations to come.  The practical "PHD" story outline will guide you in helping patients refine precious stories that contain life lessons, memories and hopes for the future. In this session, participants will quickly master skills that can be shared with patients as well as their own life work, family and community.

Learning Objectives:
1. Motivate and assist patients in the creation of a simple life story that is concise, insightful and memorable
2. Explain the biological basis for storytelling and organize life stories into a three part research based format
3. Use storytelling as a vehicle for completing life tasks, strengthening relationships and building community
