In this session, faculty will describe the unique care provided by their organization to Veterans with terminal illness. The session will also provide details on the national Life Sustaining Treatment Discussion Initiative of VHA which ensures that training, policies and procedures are in place for providers to elicit Veterans' wishes for care and to align treatments provided with goals of each Veteran.

1. Describe two unique programs the VA offers for Veterans with serious illness
2. Enhance the care of Veterans through education and collaboration with the VA
3. Discuss the importance of educating all members of the interdisciplinary team on eliciting care preferences and offering treatments that align with them
