Patients facing severe, chronic illness and their families need tremendous interdisciplinary support throughout their healthcare journey. From the time of diagnosis throughout the progression of a relentless, incurable disease, we have an opportunity to support these patients and help them live as fully as possible. Nowhere is this truer than with patients who suffer from cardiac disease. Despite this, patients with cardiac disease continue to be referred to hospice care very late in the course of their illness. This session will explore how hospices, partnering with palliative care programs (either internal to the hospice or with community colleagues) can provide expert longitudinal cardiac care to patients and their families. Using case examples and lessons learned, faculty will discuss practical strategies to develop an effective, comprehensive interdisciplinary cardiac disease state management program.
