Case management is a vital part of hospice care but there is a surprising lack of uniform competencies. Based on a set of core competencies identifed by hospice experts, a training program was developed to increase the knowledge and skills of case managers. This session will examine how this training program led to better coordination of care and services for a complex patient population while maintaining excellent quality of care and overall job satisfaction.
