Intentional design of the ‘shared care’ delivery and team management model improves organizational outcomes. The design, development, implementation and evaluation require interdisciplinary involvement, enthusiasm, accountability, measurement and conflict resolution skills. Hospice of Palm Beach County will describe its version of ‘shared care’ both as a care delivery and team management model to navigate a successful course through the sea of change.
