From the Perception of Persecution to Performance Improvement: How We Handled Medication Variances in Our Hospice Houses

Thursday, April 7, 2011: 10:15 AM-11:45 AM
Primary Presenter:
Stephen A. Leedy, MD, FAAHPM
Co-presenters:
Mary Heath, RN, BS, CPHQ , Donna Jolly, Pharm, D and Michael Argulewicz, RN, CHPN
Area of Emphasis: Quality Assessment, Performance Improvement, Benchmarking
Learning Objectives:
1. Identify the factors that can lead to medication variances in liquid controlled medications from multi-dose bottles for oral administration in inpatient hospice houses
2. Describe the strategies for evaluating medication variances, including process mapping and root cause analysis
3. Describe protocols that can be implemented in inpatient hospice houses which will help decrease the incidence of medication variances
In this climate of increased regulatory scrutiny of controlled-substance prescribing, Tidewell Hospice undertook a critical evaluation of our approach to medication variance tracking in the administration of oral liquid controlled medications from multi-dose bottles in our inpatient hospice houses. Faculty will share the evaluative methods utilized, including root cause analysis and process mapping, discuss changes that were made to protocols in response to this analysis and reveal the surprising culprit that accounted for the majority of our oral liquid controlled medication variances.
