2. Identify the critical elements of service delivery for each model
3. Describe how health plans can use this Blueprint as a road map for buying or building a sustainable model

In June 2009, the Alliance of Community Health Plan's Medical Director's Committee convened an Advanced Illness/Palliative Care Workgroup to develop a blueprint that could be utilized by the plans as they addressed the challenge of delivering high quality, community-based advanced illness/palliative care services to their plan members. In this session, faculty will share the process and output that culminated in the creation of the Advanced Illness/Palliative Care Blueprint.
