Collecting reliable and valid data is an increasing expectation for hospice and palliative care programs. Implementation of a standardized and reliable data infrastructure among community-based hospice and palliative care (HPC) organizations allows us to more effectively assess quality of care and examine patterns of access and utilization, symptoms, and advance care planning.  In this presentation, faculty will discuss the creation of a multisite palliative care database (the Carolinas Palliative Care Dataset) that provides critical data collection and data management infrastructure to support quality assessment and quality improvement (QA/QI) in HPC.
