Hospice and House Calls: An Innovative Partnership to Reach Underserved Inner City Elderly

1. Identify the most common needs for hospice care and services in an underserved, largely African American home care population
2. Define the barriers and opportunities to synergistic hospice-geriatric care created by existing financial structures
3. Determine whether a similar model is feasible in their own areas of service
Inner city African American elders who are eligible for Medicare and Medicaid have historically been an underserved population, with substantial needs for healthcare and support.  Although both hospice and eldercare outreach programs have made advances in addressing the needs of this population, previous programs have been pieced together, without formal structure or financial accountability.  This session will describe an innovative effort to coordinate eldercare, hospice services and a palliative home care program for impoverished inner city patients and their families.
