Effective communication is at the heart of every successful hospice team. That being said, ensuring optimal communication within a interdisciplinary team is an ongoing  challenge for many hospices.  This session will explore one hospice's approach to promoting effective efficient communication between multiple members of the hospice team. Specific examples will be provided, including implementation as well as the challenges encountered and how they were overcome.
