1. Describe how the care plan can be used to understand and identify the key processes that need to be changed to improve the quality of care
2. Identify types and areas of documentation in the patient record that can be used as sources for information to identify care processes that can be related to FEHC results
3. Describe how interpretation of FEHC survey results and care planning can be a synergistic process and how that process can be integrated into a hospice’s quality improvement program
The FEHC survey provides the foundation for the evaluation of a hospice’s overall quality of care. Once poor performance on a FEHC domain is identified, additional information needs to be collected to understand what key processes of care should be targeted for improvement. In this session, faculty will demonstrate how to review care plan and patient record documentation to understand which key processes to focus on in a performance improvement project aimed at improving FEHC scores. Additionally, faculty will discuss how to establish ongoing synergy between FEHC survey results and the care planning process.
