The current health care delivery system is not designed to provide needed services to seriously ill patients with Stage IV chronic illness who are not eligible or do not want hospice services nor support for their stressed caregivers.  This gap in the current care delivery system leads to poor quality, unconscionable suffering, and unsustainable cost.  Learn how Hospice of Michigan: 1) created an innovative delivery model that is improving quality outcomes for patients, supporting stressed family caregivers, and is consistenly saving 30 percent net health care costs for the serioulsy ill population; 2) implemented a national advanced illness managment company; and 3) developed a unique revenue stream to pay for the program.  Current results of the program will be presented.
