What is important to payers? How do I work with payers to build or expand my community-based program? In this new era of bundled payments and risk-sharing arrangements, it is critical that program leaders understand payers and the actions payers are taking to integrate palliative care principles and practices into care management programs, benefits, and payment models. Through case examples, this session will provide practical tips on how to work with payers to build a sustainable community-based program.
Learning Objectives:
1. Identify three innovative benefits or services offered by payers to support patients with serious illness and their caregivers
2. Identify three essential elements of effective payer/provider partnerships
3. Describe two measures important to demonstrating impact and value
