Paying for Outpatient Palliative Care, Making the Case For Cost Avoidance

This interactive workshop will outline four key elements of developing a financially sustainable outpatient palliative care program: identifying the population; the delivery of care; managing the risk; and measuring the outcomes. After a brief overview of key concepts, the specifics about patient population, clinical models of care, collaboration with physician practices, risks of managed care, defining the outcomes and metrics measurement will be detailed utilizing a case study methodology. Each of these components will be reviewed along with the actual outcomes achieved by Providence Health & Services in California providing participants with a road map of how to apply the learnings in their own organizations.
