Now that CMS is reimbursing physicians to discuss advance directives with patients, a better opportunity exists to schedule those discussions. Both physicians and patients can use encouragement to be proactive about this important issue, as well as education on how to make the discussion most productive. This session, presented jointly by a board-certified physician and marketing professional, will explore training, messaging and communication tools to prompt meaningful advance care planning discussions between physicians and patients – and how hospice organizations can better collaborate with physicians to facilitate and support the conversation.

Learning Objectives:
1. Identify questions both patients and physicians should prepare to have a productive discussion about advance directives
2. Identify gaps of knowledge and training physicians may have in effectively leading advance directive discussions
3. Indicate messaging and communication tools to encourage and facilitate advance directive discussions
