Breaking from Tradition: Preventing Hospital Readmissions and Managing Chronic Illness in a World of Value-Based Care

Hospice of the Bluegrass (HOB) provides hospice care to over 4,000 patients annually as well as operates a variety of affiliated programs. Currently, HOB is participating in two CMS Innovation Center demonstrations including the Community-based Care Transitions Program (CCTP) and the Medicare Care Choices Model (MCCM). In this session, participants will learn how a community-based organization utilizes the core competencies and skills to prevent hospital readmissions and manage chronic illness, forms deep partnerships within health care sector to develop programs that lower cost and improve outcomes and utilizes risk stratification tools to identify post-acute transition interventions.
