Evaluating and Planning for Discharge

Live discharges - only in hospice does that make sense. Hospice IDGs often struggle with finding the balance between eligibility and discharge planning which requires the IDG to continually be vigilant for signs of improvement that may lead to discharge. This session will provide an overview of the all Medicare live discharge regulations with a deep dive on discharge planning when no longer eligible. Since clinical assessments and documentation are key to effectively evaluating eligibility status, these areas will be addressed. Faculty will provide a framework for evaluating and discharge planning when a patient may no longer be eligible for hospice.
