This session will focus on providing care in last seven days of life and how each member of the team contributes to improving the experience of the patient and family during that time. In the context of public reporting and the new hospice Medicare payment model with the service intensity add-on, this presentation will offer innovative strategies to create staffing models that manage the operational and logistical challenges of the tracking of time and discipline while maximizing presence at the bedside and supporting the caregiver. Faculty will address knocking down the silos that isolate team members and explore not only the care provided directly at the bedside, but also the contributions from the education, communication, and operational team members whose contribution helps to make the interdisciplinary care of the patient and family possible.
