PACE is a program designed to meet the needs of the frail elderly and their families. Hospices offer strong foundations upon which to develop a PACE program to serve their communities. This session is designed to provide a broad overview of the key tasks on the journey to successfully opening and operating a PACE program. By sharing the lessons learned, the presenters hope to make the path easier for other hospice sponsors of PACE programs.
