3. Identify three key strategies for collaboration between hospice and palliative care organizations and ACOs
This session will describe opportunities presented by healthcare reform to expand hospice and palliative care providers' role in caring for patients while minimizing hospital re-admissions and emergency care. Specific strategies to be implemented by hospice and palliative care providers that support the accountable care structure of hospitals, primary care physicians and long term care facilities with which they work will be presented.
