2. Explain the steps to engaging other independent health care providers
3. Understand how to assess what "service gaps" exist within your hospice and the greater community
This session will outline the steps to successful collaboration along the continuum of care within a community of independent health care providers. Consistent with best practices in project management, the faculty will outline key steps and stumbling blocks to creating a community coalition. In addition, the importance of creating a culture open to innovation and moving it from “the way it has always been” to “the way it could be” thinking will be explored.
